Travel Vaccinations
If you require any vaccinations relating to foreign travel you may need to make an appointment with the practice nurse to discuss your travel arrangements. Please collect a Travel Health Questionnaire from the surgery to complete and return as soon as possible, which should include details of the country/countries you are visiting, including specific areas, as this is required to determine which vaccinations are required. The Practice Nurse will use this information ans check your medical records to see if you do need an appointment.

It is important to make the initial appointment as early as possible, if you do need one - at least 6 weeks before you travel. Please make sure you complete the travel form, so our nurse can check your immunisation history prior to your visit. Our nurse may be able to vaccinate you at this appointment, but some vaccines are not stocked within the surgery and will need to be ordered.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS. Our reception team will be happy to discuss this with you, so please do not hesitate to ask - we are here to help.
Travel Health Questionnaire
To help us offer the appropriate advice, please call into the surgery to collect the Travel Questionnaire. This is to be completed and returned to the surgery as soon as possible as you need to have any travel vaccinations at least 6 weeks before you travel. The Nurse will look at the form to see if an appointment is necessary, as you may have already received the necessary vaccinations. If an appointment is required, the surgery will contact you to make appointment. If you are travelling in less than 6 weeks we will not be able to vaccinate you and you may be directed to an independent travel clinic.
